Acne scars come in shapes, and the shape decides the treatment. Rolling, boxcar and icepick scars respond to completely different things — which is why a course of the wrong one produces almost no change and a course of the right one is transformative.

What is actually happening

A scar is collagen that healed wrong.

When inflammation reaches the deeper dermis, the repair is not a rebuild — it is a patch. Sometimes too little collagen is laid down and the skin sinks; sometimes too much, and it raises. Once that patch has matured it does not remodel on its own.

Rolling scars are broad, soft depressions tethered from below by fibrous bands. Boxcar scars have sharp vertical walls, like a small crater. Icepick scars are narrow and deep, sometimes reaching well into the dermis. Most faces have a mixture.

This is also why flat marks are not scars. The brown or red patches left behind after a spot heals are pigment and blood vessels, not damaged collagen — they fade over months and they respond to entirely different treatment. Being told the difference saves people from treating something that was going to resolve anyway.

Every effective scar treatment works the same way: create a controlled injury deep enough to trigger a real remodelling response, then let the body rebuild over months. There is no version of this that happens quickly.

How we would sequence it.

The acne has to be under control first. Treating scars on skin that is still actively breaking out creates new inflammation in the same place, and you end up chasing your own tail. If there is active acne, we clear that first, however impatient it feels.

For mostly rolling scars with some laxity, Vivace is the better instrument — it reaches the tethering and firms the surrounding skin in the same session. For shallow, widespread texture across the cheeks, SkinPen does the job with a much easier recovery.

Photographs at the start, always. Scar improvement happens over six months and human memory is a genuinely poor instrument for judging it. We have had people convinced nothing changed until the day-one photograph came out.

What to expect

Sessions, spacing and the six-month rule.

Course length
Three to four Vivace sessions, or four to six SkinPen sessions. Vivace runs six weeks apart, SkinPen four — the deeper the treatment, the longer the skin needs between.
Recovery
SkinPen: pink and tight for 24 to 48 hours, makeup from day two. Vivace: flushed and slightly swollen for two days, with a sandpaper texture for about three.
When to judge it
Six months after the final session. Collagen remodelling continues long after the course ends, and the improvement at month six is substantially greater than at week six.

What people ask about acne scarring.

How much improvement is realistic?

Meaningful but not total, and anyone claiming otherwise is overselling. A full course typically softens scarring substantially rather than erasing it — enough that it stops being the thing you see first and stops catching the light. Rolling and boxcar scars respond best; deep ice-pick scars respond least. We photograph at the start because gradual improvement is genuinely hard to judge from memory.

Can acne scars be completely removed?

Realistically, no — the goal is substantial improvement, not erasure. A well-run course typically improves the appearance of rolling and boxcar scarring by a meaningful degree over six months, enough that it stops being the thing you see first. Anyone promising complete removal from microneedling is overselling it.

How many microneedling sessions do I need for acne scars?

Four to six SkinPen sessions four weeks apart, or three to four Vivace sessions six weeks apart. Scar work needs more sessions than general skin rejuvenation because you are asking the dermis to remodel structure rather than simply improving surface quality.

Which is better for acne scars, SkinPen or Vivace?

Vivace reaches deeper and adds tightening, which makes it the stronger choice for rolling scars and for skin that has also loosened. SkinPen has an easier recovery, costs less per session and does excellent work on shallow, widespread texture. The scar shape decides it, which is why we look before recommending.

Are the red or brown marks left after acne actually scars?

Usually not. Flat marks with no texture change are post-inflammatory pigmentation or erythema — pigment and dilated vessels, not damaged collagen. They fade over three to twelve months on their own and respond well to peels, IPL or Aerolase. Treating them as scars means paying for the wrong treatment.

Will microneedling make my acne worse?

It can, if it is done while the acne is still active — needling across an inflamed area spreads bacteria and provokes new lesions. That is precisely why we clear the acne before starting scar work. On calm skin, microneedling does not cause breakouts beyond an occasional transient one in the first week.
